Discharged Tab
The Discharged Tab presents a list of referrals that have been discharged by a provider. Further information is provided below:

The Discharged Tab presents a list of referrals divided into the following columns: Patient Name, NHS Number, Referring Clinician, Days Since Referral, Presenting Complaint and Status. Referras in this tab will have been discharged by the provider and will be indicated as ‘Consultation completed’ or the patient ‘DNA’ (did not attend) / ‘DNR’ (did not respond) in the status column.

Referrals may be sorted in ascending or descending order by clicking on each column title. The search function located on the top left hand side of the screen enables users to locate a patient in this tab by entering either: First Name, Surname, Date of Birth (DOB) or Unique Reference Number (URN). 

By clicking on the patient name, the full history of the referral will be presented including notes/interactions recorded by user and clearly date-stamped. Date of acceptance will be displayed in all instances. Details such as consultation information and images may be viewed.

To save a copy locally, please select ‘PDF’ to download a full copy of the referral. You may also import the file onto your patient record system or print as required.

Please note: Patients discharged under ‘DNA’ or ‘DNR’ may only be resent following the creation of a new referral in accordance with locally agreed guidance. Columns can be adjusted by clicking and dragging to expand any compressed information.
