Completing Reason/s for Referral

Each reason for referral requires specific information and is dynamic according to responses provided. Once ALL mandatory fields have been completed and where required, images uploaded, users may click ‘Next’ to proceed. Further information is provided below:

  • Select the relevant tooth/teeth or area/s where a dentogram or mannequin is displayed 
  • Select an option from the drop-down lists provided
  • Certain responses may require further clarification; please ensure that you answer any follow up questions as they are mandatory
  • Click the ‘Add’ button to upload an image where prompted. Ensure that all images have a date of exposure
  • Enter any other relevant information into the free-text field where prompted
  • Click the purple ‘Next’ button when all mandatory information has been provided
  • In some cases, if the information provided does meet local criteria, a prompt will advise a user to click ‘Remove Referral’ located on the top right hand side of the reason for referral box.

Please note:

The content, logic and mandatory fields within each reason for referral may vary based on the information provided within the medical history and patient modifying factors section along with the patient's demographics (including age and location of registered GP practice).

The responses provided will indicate the level of clinical complexity and eligibility for referral; in some instances, prompts will offer further advice. With this in mind, and to ensure full accuracy, please treat this section with extra care and enter from the drop down boxes wherever possible as the logic on the system does not recognise information that has been uploaded or provided within free-text boxes.
